
The Social Services Department consists of experienced, multi-lingual Community Health Advocates and licensed Social Workers. We offer comprehensive services to our patients assisting them with their personal needs and working in collaboration with their health care providers to offer holistic, quality care.
Community Advocacy and Outreach
Concrete Services: including Resources and Referrals for housing, food and clothing assistance, utilities, day care, parenting programs, GED and employment, mental health and substance abuse resources, financial and legal assistance, immigration assistance, domestic violence resources and many additional community resources.
