Home Care

The Home Care Department's mission is to provide quality, comprehensive and coordinated home care services to the residents of Dorchester and surrounding areas. Our staff has years of experience coordinating and delivering services needed to meet the patient's medical, physical and family needs.

Upham's Home Health Care Program is part of the Upham's Corner Health Committee, Inc., a non-profit organization. The program is certified by the Massachusetts Department of Public Health and by the Health Care Financing Administration.

We offer the following services:

Home Health Programs

These programs help people stay at home when faced with serious health problems. For each program, a team of health care providers and support staff is brought together to meet the patient's unique medical, physical and family needs.

  • Frail Elderly and Disabled Adult Services
    -long-term care for those with acute/chronic health problems or terminal illness

  • Short-Term Rehabilitation
    -designed for people who need care only on a short-term basis, such as recovery from an injury or surgery.

  • AIDS Services
    -interdisciplinary care to meet the emotional and physical needs of patients and significant others at any stage of HIV infection.
