Behavioral Health Counselor
Part-time Behavioral Health Counselor opportunity available in the Behavioral Health department at Upham’s Corner Health Center. This position is responsible for delivery of therapeutic services to registered patients in a dynamic, multicultural community health center. Responsibilities include:
- Providing psychosocial assessments and behavioral health treatment to individuals, couples, family and groups.
- Providing crisis intervention and information about community resources and services to UCHC clients referred by PCP.
- Maintaining individual caseload and deliver group therapy for children and adults.
- Completing all appropriate paperwork and documentation in compliance with Behavioral Health department policy and procedures.
- Perform additional duties of the Behavioral Health department as needed.

QUALIFICATIONS: Masters Degree in therapy, counseling or social work and current Massachusetts Therapist or LICSW license. Bilingual in Spanish is preferred.

WIC Breastfeeding Peer Counselor
A WIC Breastfeeding Peer Counselor is a paraprofessional staff person who provides basic breastfeeding women, under the direction and supervision of the Local Agency WIC Breastfeeding Counselor.
QUALIFICATIONS:
- Minimum of High School Diploma or equivalent (GED)
- Is enthusiastic and positive about breastfeeding, and wants to help other mothers enjoy a positive experience.
- Has breastfed at least one baby for 6 months, at a minimum, within the past 5 years.
- Is a previous WIC participant.
- Is available to work in WIC office.
- Has telephone and is willing to make and receive calls at home, outside of usual WIC clinic hours.

Medical Records Clerk
In this full time position, you will be responsible for processing medical records and maintaining and organizing medical records files. You will utilize clinic appointment lists to pull appropriate records and deliver charts to clinic and re-file returned records. Also, you will process and file lab results and correspondence in appropriate charts. Additionally, you will prepare color-coded tracer folders for circulating charts, answer the telephone and respond to requests for records and copying. Check through all records to remove inactive charts to long term storage.
QUALIFICATIONS: High school diploma/GED and strong English reading, writing and computer skills are required. Two years administrative or medical records experience and excellent filing and communication skills. Bilingual in English and Spanish required.
HOURS: 35hrs/week
Monday – Friday

VIP Block Captain
The VIP Block Captain will work with residents of the identified VIP neighborhood to develop, implement and sustain a local coalition that will set up a community based strategy that prevents violence under the direction of your supervisor.
- Assist with the development of the coalition, including recruiting resident volunteers and engaging residents in monthly coalition meetings
- Participate in outreach activities directly working with the community residents, including but not limited to, door-knocking to resident homes, disseminating VIP and related event information, engaging residents in social media networks
- Attend VIP coalition meetings and activities
- Host or Co-host 6 resident house parties per year
- Participate in planning meetings held by Boston Public Health Commission, partner agencies and other coalition leaders.
- Contribute to the Violence Prevention Plans, reports, data collection information, invoices, and any additional information requested by your supervisor, Boston Public Health Commission
- Work collaboratively with other VIP coalition teams in the spirit of alliance to service and empower the community
- Utilize a data collection method to ensure accurate tracking of VIP neighborhood coalition activities that includes a set of common indicators across VIP neighborhoods
- Follow all UCHC policies and procedures
- Meet with your supervisor, when requested, to report on your success and challenges in fulfilling your assigned tasks. Other tasks determined by supervisor
QUALIFICATIONS: High school diploma or equivalent. Some working knowledge of community resources. Must be well organized and computer literate, knowledge of social media tools are a plus. Good communication skills, able to work well with a diverse group of people and can function as part of a team environment. Bilingual skills in English and Spanish/Portuguese. Must be available some evening and some weekend hours. Must be familiar with the VIP neighborhood and be a part of the VIP community.
OB Health Advocate
Under the supervision of the CHA Supervisor , this position will provide clerical support to the OB Department and Social Services dept.
- Check the +UCG in the lab and OB Department and schedule appointments with nurses for initial intake.Collect all new OB prenatal patient information.
- Ensure OB list for UCHC is updated with registered new patients and adding patients to the OB list for tracking during pregnancy and postpartum.
- Responsible for faxing and mailing all prenatal charts to assigned hospitals at the 12 week visit, 26 week visit, and 36-38 week visit as needed. (BWH is handled by Advocate, and BMC is handled through the Medical Records Department.)
- Identify prenatal patient charts by placing blue dots on charts and forwarding them to the Medical Records Department.Create a Dummy Folder for the Medical Records Department, in replacement of OB chars in the regular record slot.
- Organize flash cards for UCHC records. (In-house by monthly record)
- Complete a data report form for UCHC and the Department of Public Health.
- Collect and electronically organize all OB statistics data; Generate reports as requested for the UCHC OB case reviews, MCH reporting, SAB, TAB, Delivery by Month, and Transfers. Update information as changes are made.
- Ensure and organize follow-up appointments with MD and Family Planning Nurse.
- Maintain and keep track of the following:
- TAB, SAB, and LMR System;Delete patients from OB list.;schedule postpartum appointments.;Ensure patients are aware of their appointments.;Attend the monthly OB meeting
- Community meeting and weekend flexibility is required.
- Notify the Medical Records Department of delivery discharge upon postpartum visits.
- Notify the Billing Department of delivery for billing purposes. Other tasks may be needed from the supervisor.

Lead Community Health Advocate
DUTIES:
- Follow-up with identified non-compliant and disconnected patients particularly regarding lead toxicity testing and treatment, to ensure continuity of care.
- Participate with planned outreach/education activities with highlighting lead poisoning prevention and treatment.
- Consult with primary care providers on lead poisoning prevention and treatment for patients and mental health clinicians on complex client situations in order to provide translation as needed.
- Promote and educate community around health care services, and assist in patients accessing programs.
- Provide and link patients with community resources and referrals (negotiate interagency and intra-agency contacts).
- Identify needs, and provide general case management including but not limited to ESL, WIC, DPW, Legal Aid.
- Provide cross-cultural translation services and education with multidisciplinary team. Including but not limited to: HIV/AIDS, lead poisoning, WCC. Provide both verbal and written translation.
- Conduct home visits as required for primary care patients.
- Document all services as required and maintain statistics.
- Attend in-service trainings and outside seminars, as indicated.
- Attend weekend health fairs as needed.
- Attend community meetings once a month.

QUALIFICATIONS: Bilingual in English and Spanish; High school Diploma or equivalent.

Dentist
Full – Time or Part – Time Position. Under the general direction of the Dental Director: provide dental care for patients by diagnosing and treating diseases, injuries, and malformations of teeth and gums.
- Take and diagnose radiographs.
- Perform clinical examination and formulate treatment plans.
- Perform all phases of restorative dentistry and prosthetics.
- Perform endodontic therapy or refer therapy if best handled by a specialist.
- Perform periodontal therapy including minor surgical procedures.

Occupational Therapist and Physical Therapist
Part time positions available (flexible schedule) for licensed physical and occupational therapist with 2 to 3 years working with the elderly or chronically ill, cognitively impaired or frail geriatric population. Responsibilities will include collaboration on patient care plans with physicians and direct patient care. Educating patients and their families as well as staff in on treatment plans, home evaluations, and participating in clinical team meetings.
Primary Care Physician
Provides medical care to families, including health maintenance, episodic care, and chronic disease management.
- Provides primary medical management for patients.
- Delivers patient care and patient education in the manner that is appropriate to patient's age, physical ability, educational level, and culture.
- Conducts health assessments and evaluates the needs of patients in the primary care clinic.
- Conducts physical exams, orders and/or performs laboratory and diagnostic tests, and makes clinical judgments regarding results.

Part-Time Registered Nurse
Part-time weekend nurse working with UESP. Responsible for the evaluation of patients as well as providing monitoring, follow-up and skilled nursing care for the patients assigned to him/her.
To execute physician orders as required and to be professionally responsible to the nursing supervisor.
- Cover day center hours when needed.
- Provide weekday episodic and scheduled skilled nursing visits.
